The first step in becoming a SIMS client is to complete the Client Intake and Client Consent forms below. Filling out the forms only takes a few minutes, and you can submit them online.

After you submit your forms, a member of the SIMS clinical staff will contact you within 48 business hours to verify your eligibility and set up an enrollment appointment with a licensed SIMS clinician.
Eligibility requirements for SIMS services:
To be eligible for SIMS services, you must have earned some degree of income through the music industry in the past 12 months. You must also reside in one of the following counties: Travis, Bastrop, Williamson, Burnet, Blanco, Hays or Caldwell.
SIMS also covers spouses, partners, and dependents of eligible musicians or music industry professionals.

You will meet over the phone or in person with a SIMS staff clinician. This confidential conversation helps your SIMS clinician understand your individual needs and identify the types of care that would best support you. Your SIMS clinician will give you the names of suggested providers. The clinician will also help you decide on a manageable co-pay amount that works with your income. Co-pays are determined on a sliding scale.
