Eligibility

You may qualify for behavioral healthcare services through SIMS if you meet the following criteria: 

1. RESIDENCY: You can demonstrate proof of residency in one of the counties SIMS serves (Bastrop, Blanco, Burnet, Caldwell, Hays, Travis, and Williamson). If you are currently unhoused, please contact us for more information at 512-494-1007 or clinical@simsfoundation.org. Being unhoused will not be a barrier to care. 

EXAMPLES OF PROOF OF RESIDENCY WE ACCEPT: 

• Valid ID/driver license 

• Lease with full date visible 

• Bills with full date visible 

• Current HAAM card
   

2. INDUSTRY WORK: You can demonstrate income earned in the music industry in the past 12 months or are a dependent family member of a person who can demonstrate income earned in the music industry in the past 12 months. 

EXAMPLES OF ACCEPTED PROOF OF INDUSTRY WORK FOR MUSICIANS: 

• Professional website that lists all gigs and includes the full date of the performance and the venue 

• Social media accounts that list all gigs and includes the full date of the performance and the venue 

• Copy of an invoice from an online outlet where the individual’s music is sold, such as Bandcamp. The full date of the payment, the amount of the payment and the item for which the individual is being paid must be clearly visible 

• Copy of a paycheck that includes the full date, the performance for which the individual was paid and the name of the business making the payment 

• Copy of a cash app payment (such as Venmo) from a verifiable account that includes the full date, the performance for which the individual was paid and the amount paid 

• Current HAAM Card 

EXAMPLES OF ACCEPTED PROOF OF INDUSTRY WORK FOR MUSIC INDUSTRY PROFESSIONALS: 

• Copy of a paystub for work completed that includes the full date and clearly shows the employer’s name.
• Copy of a contract for services that includes the full date, outlines the services for which the individual is being paid and the amount of payment.
• Copy of Tax Return
• Copy of a cash app payment (such as Venmo) from a verifiable account that includes the full date, the work completed for which the individual was paid and
  the amount paid.

SIMS DOES NOT ACCEPT THE FOLLOWING AS PROOF OF INCOME IN THE MUSIC INDUSTRY: 

• Pictures of flyers for events 

• Videos of a performance 

• Letters or copies of cash app payments from unknown individuals 

3. UNDER 400% OF THE FEDERAL PROPERTY LEVEL (FPL): Your annual gross household income is below 400% of the Federal Poverty Level (FPL). To review the 2025 FPL chart and learn more about how/why SIMS uses this guideline, please see the slide below. Every SIMS member has a unique set of circumstances so the availability of financial assistance through SIMS is evaluated individually. In some instances, members may be asked to provide verification of household income.  

EXAMPLES OF ACCEPTED VERIFICATION OF HOUSEHOLD INCOME: 

• Most recent tax return 

• Social Security or disability benefits letter 

• Bank statements showing regular direct deposits of income (past 60 days) 

• Copy of application for insurance through Affordable Care Act (ACA)
